      Before we begin, please acknowledge the following:

      By scheduling this appointment, you are requesting a mobile collection for convenience.

      You have the option to have your specimen collected at a Quest Patient Service Center of your choice. By proceeding with this At-Home Collection request, you agree to pay Quest at Home directly a $55 per visit mobile collection fee via credit card at the time of scheduling.

      Patients are responsible for arranging payment for the collection fee via credit card at the time of scheduling.

      Quest at Home cannot perform mobile collections on patients under 12 years of age.

      At this time, the service is available for purchase for patients who are self-pay or have commercial insurance (not Medicare/Medicaid).

      Quest at Home will not bill health insurance plans for mobile collection services.

      Quest at Home cannot perform mobile collections on patients under 12 years of age.

      Quest at Home in-home collections are designed to provide convenience. Medically appropriate home draws for patients with limited mobility may be available from Quest Diagnostics and other providers outside this program.

      All patients may choose to have their specimens collected at a Quest Diagnostics Patient Service Center, and by proceeding to request an in-home appointment you agree to pay Quest at Home’s mobile collection fee directly.
